What is a Fracture? (Stress vs. Traumatic)

Many people believe there is a difference between a “break” and a “fracture.” In medical terms, they are the same thing: a loss of continuity in the bone. However, fractures in the foot and ankle generally fall into two categories:

  1. Traumatic Fractures: These happen suddenly due to a specific event, such as twisting your ankle, dropping a heavy object on your foot, or tripping on a curb. The bone breaks due to immediate force.
  2. Stress Fractures: These are tiny, hairline cracks in the bone caused by repetitive force or overuse. They are common in runners, athletes, or people who spend all day on their feet on hard surfaces. Unlike traumatic fractures, the pain from a stress fracture often starts gradually and worsens over time.

Symptoms: Is My Foot Broken or Sprained?

It can sometimes be difficult to distinguish between a severe sprain and a fracture without an X-ray, but there are distinct warning signs of a break:

  • Immediate, throbbing pain: Pain that is sharp at the moment of injury and settles into a deep ache.
  • Inability to bear weight: If you cannot take four steps without significant pain, it is highly likely you have a fracture.
  • Swelling and bruising: Significant swelling usually occurs within minutes to hours. Bruising (black and blue discoloration) may appear quickly or settle into the sole of the foot later.
  • Deformity: If the foot or ankle looks crooked, dislocated, or abnormal in shape, the bone is likely broken and displaced.

At-Home Care: Immediate First Aid (R.I.C.E.)

Important: You cannot “heal” a broken bone at home. Home care is strictly for stabilizing the injury and managing pain until you can get to our office. If you suspect a break, follow the R.I.C.E. protocol immediately:

  • Rest: Stop walking on the injured foot immediately. Do not try to “walk it off.” Use crutches if you have them.
  • Ice: Apply an ice pack (wrapped in a thin towel) to the injured area for 20 minutes at a time to reduce swelling and numb the pain. Do not apply ice directly to the skin.
  • Compression: Gently wrap the foot or ankle with a soft bandage (like an ACE bandage) to control swelling. Do not wrap it too tightly, as this can cut off circulation to your toes.
  • Elevation: Lie down and prop your foot up on pillows so that it is higher than the level of your heart. This uses gravity to drain fluid away from the injury and reduce throbbing.

When to See a Doctor

Because the foot contains 26 small, intricate bones, ignoring a fracture can lead to chronic arthritis, permanent deformity, or non-union (where the bone fails to heal together).
